Required forms for all NEW and RETURING players:
Copy of player’s birth certificate (NEW player's only)
KYSA Medical Release Form (this form must be notarized)
Current player photo, 1 x 1
Elizabethtown Sports Park Liability Waiver
Player/Parent obligation form
